If your insurance is not listed above, please call the 1-800 number on the back of your insurance card to see if we are in-network.
If we are not in-network for your insurance, you can still see Dr. Tole as an out-of-network specialist and submit the charges to your insurance company. Contact your insurance company for details.
Copays are dependent on each patient’s insurance. Contact your provider for expected copay costs.
Payments are expected at the time of service. East Alabama Allergy and Asthma accepts cash, card, or check payments.
A fee of $50 is charged for patients who fail to show at the time of their appointment. Patients are considered late 15 minutes after their scheduled appointment time and will have to reschedule their appointment.
